How to Set Up an Evaluation
Step 1:
The family can call PTC at 713-772-1400 or email Nikki Woods to set up an evaluation.
Step 2:
PTC collects the patient’s demographics (Contact Information, Patient’s Diagnosis/Condition and Insurance).
Step 3:
PTC verifies benefit coverage with the patient’s primary insurance. If necessary, PTC will help the family obtain a referral authorization from the primary care physician. PTC will discuss the benefits quoted with the patient’s parent/guardian.
Step 4:
Each patient is carefully paired with a therapist who will then call the patient’s family to set up an appointment for an evaluation.
Step 5:
The patient receives an initial evaluation and is given recommendations. A “Standing Appointment” is made.
